Dependancy Visa for a Malaysian Wife Married to a UK (PR)

I am a Malaysian and got married to a Indian National who is at the moment working in UK for the last 9 years and holds a PR status. I am now in UK and will be back in Malaysia to apply for the Dependancy Visa. Should my husband accompany me to appy for the visa in KL. I would like to know the conditiions I should fulfill to get the visa. I would also like to know the payment towards the visa application. I wish to know as to the duration taken to approve the visa. Thank you.
